Legacy Dependent Using the Hazlewood Act
New Legacy Dependent Hazlewood User
First, you should attain legible copies of the following documents:
- Veteran's DD-214 (Member 4)
- Copy/Screenshot of Hazlewood hours used by both you and the veteran (even if no Hazlewood hours have been used). Both you and the veteran must register at Hazlewood online
- Proof of Dependence. (Birth Certificate, or veteran's most recent IRS Tax Transcript listing you as a dependent. If you are not the veteran's biological child, then adoption certificate, or veteran's IRS Tax Return Receipt, or your birth certificate plus veteran's marriage certificate is acceptable).
- Provide proof of veteran's Texas resident. (A copy of the most recent utility bill with the veteran's name and Texas address is preferred; a valid driver's license is acceptable). If the veteran is deceased, please provide a copy of the veteran's death certificate.
- If the veteran served after 9/10/2001 and transferred unused Post 9/11 GI Bill® months to you, provide proof of eligibility or ineligibility for Post 9/11 GI Bill from the Department of Veterans Affairs. If not, disregard this step.
Then, upload those documents, and complete your Hazlewood Application.
Continuing student
- Proof the veteran is a Texas resident (driver's license, state identification card, voter registration card, or a utility bill). If the veteran is deceased, please provide a copy of the veteran's death certificate.
- If you are using any GI Bill benefit, submit proof of remaining entitlement.
Then, upload those documents, and complete your Hazlewood Application.
