Accommodation Letter Request

Steps for notifying your instructors of your approved academic accommodations:

1. Fill out and submit this form.
2. Come by SSD to pick up your letters. (The letters will generally be ready within 3-5 business days from the request.)
3. Once you obtain your letters from SSD, meet with your instructors to give them the letters and discuss your accommodations.
4. If you are in the College of Engineering and plan to utilitize testing accommodations through Engineering Students with Disabilities, you will need to request an accommodation letter for Beth Glenn
5. If you are in the School of Law, you will need to request an accommodation letter for Brandi Weaver only.
6. If you need a letter for a placement exam, request a letter for Kendall Tolliver.
7. If you are a student in the College of Pharmacy, please request a letter for Dr. Richard Wilcox.
8. If you are a student in the School of Nursing, please request a letter for Dr. Patricia Carter.
