Faculty Disposition of a Scholastic Dishonesty Matter

This form allows faculty members to resolve a scholastic dishonesty matter directly with a student. After both the instructor of the course and the student have completed and signed the form, please send this form and the associated evidence to Student Judicial Services. Discipline Referral for Scholastic Dishonesty

Use this form to refer a scholastic dishonesty matter to Student Judicial Services. We will then investigate and address the matter, and you will be notified when the matter has been resolved. (You must login with your UT EID and password to download this form.)
