Reporting an Incident to Student Judicial Services

Academic Integrity Violations

This form should be used by faculty members to report academic integrity violations. For more detailed information about this process, please read the procedures for reporting an academic integrity violation. Students who wish to report an incident of academic dishonesty should speak with the course instructor or contact our office at (512) 471-2841 or sjs@austin.utexas.edu.

Non-Academic Incidents

Non-academic incidents may be reported to Student Judicial Services by completing our online referral form. If you have any questions, or if you would like to first consult with a staff member before reporting the incident to our office, please contact us at (512) 471-2841 or sjs@austin.utexas.edu.
