Application Dropoff / Pickup Form (pdf)
Use this form to let the SOC staff that you'll need their help collecting applications.

Application for Registration-New Organization Form (pdf)
Starting a new organization? You'll need to bring this form, along with an Authorized Representatives Form, Legal Responsibility Form, and $10 registration fee to the Student Organization Center (SOC).

Bank Authorization Form (pdf)
Use this form to list the organization members authorized to use your organization's account at the Student Organization Bank.

Employee Identification Number (EIN) (pdf)
Employee Identification Number is required for any payment to an organization which is processed through the University. These include, but are not limited to, payments for stadium clean up, ushering at the Frank Erwin Center, etc. You may also apply for an EIN online at irs.gov.

Student Organization Campus Event Planning Guide (pdf)
The Student Organization Campus Event Planning Guide is available to help guide your organization in its event planning process so that you can have a successful event. This form will assist your organization in considering and managing the risks you could encounter in your activities.

Legal Responsibility Form (pdf)
This form is a four-part affidavit stating that: 1) the group will not participate in any hazing activities, 2) the group will only have members that are students, staff, or faculty, 3) the group discloses whether they own, lease, or otherwise control any property, and 4) the group provides a solicitation statement from the previous semester. This form needs to be signed by a currently authorized representative.

Organization Information Change Form (pdf)
Update your organization's purpose, membership, activities, add/remove officers, change your name, etc by downloading this form and returning it to the Student Organization Center.

Outdoor Event Change / Cancellation Form (pdf)
Use this form to request any changes or cancellations for a previously approved outdoor event, including changes to dates, times, or activities.

Property Form (pdf)
If your group owns, leases or otherwise controls any property, this form should be used to provide the address and location of said property. This form needs to be signed by a currently authorized representative.

Room Change/Cancellation Form (pdf)
Please fill out and turn in to the SOC counter a Room Change/Cancellation Form if you wish to change or cancel a previously approved reservation. Do not submit another room reservation request electronically or on paper to replace or alter an existing reservation. If you have already paid a weekend room fee and are canceling within two working days of the event you may request a refund. For change requests, list all current reservation information as well as the changes wanted on the form, turn it in to the SOC and return to pick up a copy of the processed form after it has been approved by the SALD office.

Room Reservation Request (online form)
This form is used to book new reservations in general purpose classrooms for registered student organizations only. Any changes that need to be made to a reservation after it has been processed must be submitted on a Change and Cancellation form (pdf). Do not submit another reservation form to alter an existing reservation since every form submitted will be processed and will become an additional reservation. Please note that any reservations starting at 5:00pm on Fridays through all day Sunday carry fees of $5.00 per room/per day that are payable in advance at the Student Organization Center in order to receive your room confirmation.

Sales Tax Permit (pdf)
Use this form if your organization is planning on selling goods/or services (This may include Ticket Sales to an event).

Sponsorship Form (pdf)
Use this form to apply for your organization to become officially sponsored by a UT department or college.

Warehouse Information Sheet (pdf)
This form contains Events and Moving Services warehouse instructions and information for student organizations that have an approved work order through SALD to pick up and return items.

Work Order Change / Cancellation Form (pdf)
Use this form to request any changes or cancellations for a previously approved work order, including changes to dates, locations, or equipment additions. Please turn in the form to the Student Organization Center and return for the confirmation after it has been approved.
