Application Dropoff / Pickup Form If your organization accepts membership applications, the Student Organization Center (SOC) can serve as your centralized application drop-off location. To use this service, complete the online form. Please note, the SOC cannot accept any money that may accompany an application.

Application for Registration-New Organization Form Starting a new organization? Apply online.

Authorized Representative Updates (online; UT EID required) A current authorized representative of a registered student organization can update the authorized representatives by completing this online process.

Employee Identification Number (EIN) Required for any payment to an organization that is processed through the university. These include, but are not limited to, payments for stadium clean up, ushering at the Frank Erwin Center, etc.

Governing Documents (pdf) Adopting a constitution and bylaws helps student organizations function in an orderly manner. Visit our Creating Effective Organizations page for helpful information on how to create governing documents for your organization.

Mailbox Stuffing Request Form The Student Organization Center staff will stuff a flyer produced by registered student organizations, University academic and administrative units or off-campus businesses and organizations into each of the active registered student organizations mailboxes.

Organization Information Change Form Update your organization's purpose, activities, etc. by downloading this form and returning it to the Student Organization Center. Changes to your authorized representatives can be also be made online with the Authorized Representatives Updates.

Sponsorship Form (online; UT EID required) Use this application if you would like your organization to be officially sponsored by a UT Austin department or college.

Domestic Travel Information Pack (pdf) Step-by-step travel instructions for registered student organizations traveling domestically.

Fire Prevention Services Special Event Form (online; UT EID required) Required for any registered student organization planning to host a special event on campus, whether big or small, to help ensure that proper fire safety and prevention procedures are followed.

General Purpose Classrooms (pdf) Lists potential meeting spaces on campus. Use this as you plan to reserve rooms on campus for your meetings and events.

Outdoor Event Change / Cancellation Form (pdf) Use this form to request any changes or cancellations for a previously approved outdoor event, including changes to dates, times, or activities.

Parking and Transportation Special Event Request Form Required for any registered student organization hosting an event on campus in which parking guards and/or reserved parking will be needed for participants, and which could potentially disrupt campus parking.

Registered Student Organization Campus Event Planning Guide (pdf) Facilitates your organization's event planning process to help ensure that your event is a success. Assists your organization in considering and managing the risks you could encounter in your activities.

Room Reservation Request (online, for registered student organizations only) Use to book new reservations in general purpose classrooms. Changes that need to be made to a reservation after it has been processed must be submitted via e-mail to Rachel Dybala. Each form submitted will be processed and will become an additional reservation, so do not submit another reservation form to alter an existing reservation. Reservations starting at 5 p.m. on Fridays through all day Sunday carry fees of $5 per room per day, and must be paid in advance at the Student Organization Center in order to receive your room confirmation.

Sales Tax Permit (pdf) Use this form if your organization is planning to sell goods or services (including ticket sales for an event).

Warehouse Information Sheet (pdf) Events and Moving Services warehouse instructions and information for student organizations that have obtained an approved Student Activities work order to pick up and return items.

Weapons Approval Form (pdf) Required for any student or student organization wanting to possess, use or display firearms, facsimile firearms, ammunition, explosives or other weapons on property owned or controlled by UT Austin.

Risk Assessment Matrix (pdf) A self assessment tool that student organizations can use to assess the risks that may be associated with a particular event. Provides step-by-step instructions to determine the level of risk and what management strategies you and your fellow student leaders can use to reduce the level of risk.
