Event Planning Form (pdf)

The form helps guide your organization in its event planning process. It is a recommended tool for all events, and it is required for any group reserving outdoor space on-campus.

Fire Prevention Services Special Event Form (online; UT EID required)

This form is required for any registered student organization planning to host a special event on campus, whether big or small, to help ensure proper campus fire safety and prevention procedures are followed.

Food Distribution Form (online; UT EID required)

This form is required anytime an event is planned where Potentially Hazardous Food is going to be distributed (whether prepared by students or a caterer) to others on campus such as a student organization wanting to grill and distribute burgers to passersby at the West Mall.

Governing Documents (pdf)

Use this form to assist your organization in writing a Constitution and/or Bylaws.

Parking and Transportation Special Event Request Form

This form is required for any student organization hosting an event on campus that will require reserved parking for participants, involve the need for parking guards, and/or potentially disrupt campus parking as a result of the event.

Registered Student Organization Manual (pdf)

This manual provides tips and resources available to your organization regarding trademarks, event planning, risk management, banking and finances, etc.

Sales Tax Permit (pdf)

Use this form if your organization is planning on selling goods/or services (This may include Ticket Sales to an event).

Sponsorship Form (pdf)

Use this form to apply for your organization to become officially sponsored by a UT department or college.

Travel Information Packet (pdf)

This form includes step-by-step travel instructions for Registered Student Organizations.

Weapons Approval Form (pdf)

This form is required for any student or student organization wanting to possess, use, or display firearms, facsimile firearms, ammunition, explosives, or other weapons on property owned or controlled by the University.
